2011 Winter Youth Break-Away Flyer and Registration Form

This is the simple paper form that you can download and print off. You can then fill it out by hand and send it into our office with your payment. Register by December 20 to avoid a late fee.

Not sure you can afford camp? We want everyone to be able to come. Fill out a this application for financial assistance from our Bruce Roberts Memorial Scholarship Fund.

Challenge Course Consent Form

This consent form needs to be filled out by all guests participating in a session of the Community Challenge Course. Please send or fax to our camp office, or bring completed forms with you for your Challenge Course session.

FORMS NEEDED FOR COUNSELORS AND VOLUNTEERS

Voluntary Disclosure form

This form is required for all staff, volunteers, and work team members and must be updated yearly.

Camp Staff Registration

This registration is required for all volunteers and staff who will be working during our summer programs (i.e. counselors, deans, summer volunteers).

Health Form

This health form is also required for all volunteers and staff who will be working during our summer programs (i.e. counselors, deans, summer volunteers). Work team members should also fill out this form.

APPLICATIONS FOR INTERNS & CITS

Counselor in Training (CIT) Application

High School students between the ages of 16 and 18 can download this form to apply for our summer Counselor in Training program at Birchwood Camp. Students who participate develop strong leadership skills, get to spend extra time at camp, AND they get 50% off tuition to their own Birchwood Camp High School camp program.

Volunteer in Mission Team Informational Packet

Interested in bringing a work team to Birchwood Camp? Download this packet for all the information you’ll need to learn about the Camp and planning a trip to Alaska.  The packet also includes an application for your work team to serve at Birchwood Camp.

Individual Volunteer Application

This form is for individuals that are coming to Birchwood Camp to serve as volunteers for a period of time (usually during our summer season). Please submit this form to the camp office, so that we can learn more about how you might fit into the ministry of Birchwood Camp.
